What’s the Difference Between Dry Needling and Acupuncture?

At Precision Physical Therapy, we often get asked about the difference between dry needling and acupuncture. While both use thin, sterile needles, they are actually quite different in purpose, training, and results.

The Purpose of Dry Needling

Acupuncture is a traditional Chinese medicine technique that focuses on restoring energy flow (known as “Qi”) through specific points in the body. It’s based on ancient meridian pathways and is used to promote overall balance, wellness, and relaxation. Both acupuncture and dry needling use thin needles, but their purposes and theoretical bases differ.

Dry needling, on the other hand, is a modern, evidence-based physical therapy treatment designed to relieve muscle pain, tightness, and trigger points. To describe dry needling, it is important to note that it defines dry needling as a technique that uses thin needles to target myofascial trigger points and muscle tissue. Dry needling also targets connective tissue and connective tissues, such as tendons and ligaments, in addition to muscle. It is commonly used to relieve pain associated with myofascial pain syndrome by addressing myofascial trigger points and improving function.

The Method of Trigger Point Dry Needling

In dry needling, a physical therapist targets specific muscles that are tight or overactive. Trigger point identification is often attempted through manual examination and physical examination, but these methods have limitations in reliability and validity. The needle is inserted into skeletal muscle, specifically targeting muscle fibers within myofascial trigger points. Techniques such as deep dry needling, which penetrates deeply into the muscle to elicit a local twitch response, are contrasted with superficial dry needling, which targets only the skin and subcutaneous tissue, and direct dry needling, which is a modern, evidence-based approach focused on myofascial trigger points rather than traditional acupuncture points. Intramuscular manual therapy and intramuscular stimulation are also used to target muscle fibers and skeletal muscle during dry needling. The needle creates a local twitch response, an involuntary muscle contraction that is important for confirming accurate trigger point identification and is associated with improved treatment outcomes. Dry needling aims to address electrical dysfunction and biochemical and electrical dysfunction at the motor endplate, which normalizes biochemical processes, reduces muscle tension, and increases blood flow. This process can disrupt abnormal pain patterns, improve pain control, and promote tissue healing. Patients often feel immediate relief and improved movement.

Acupuncture follows a set pattern of points along energy channels and typically does not aim for a muscle twitch.

The Results in Myofascial Pain Syndrome

Dry needling is especially effective for athletes, chronic pain, and post-injury recovery — making it a powerful tool for fast, targeted results. Dry needling treatments are used for a variety of musculoskeletal conditions, including musculoskeletal pain syndromes such as shoulder pain, neck pain, and hip pain. Dry needling treatment can help with pain reduction, relieving pain, and reducing disability, and is considered an effective treatment for certain pain syndromes. It can also target scar tissue and underlying myofascial trigger points, and is often performed by licensed physical therapists, many physical therapists, and sometimes medical doctors. Dry needling is typically part of a larger treatment plan, which may include active rehabilitation to address movement impairments. Trigger point dry needling uses dry needles, such as a filiform needle, and may involve multiple needles for optimal results.
